To be completed by the Show Secretary and attached to the shows results and returned to the BSJA Offices at Stoneleigh.

To Be Finalised and Completed by each H&S Officer for each show

To be completed by the Show Secretary. If you organisation is non-profit making please ensure you tick the box at the bottom, this will indicate if you show is to be charged VAT, if the box is left blank this will indicate that you pay VAT. This form is to be used to book show dates, please ensure when sending in this form that the correct booking fees are attached.

Must be completed and returned with all draft schedules
